This self-assessment tool is designed to help the Board of Directors assess its current performance and identify priorities for future governance discussions and decisions.

The survey should take 15-20 minutes to complete. All responses will be kept anonymous and results will be reported in aggregate at the annual Board of Directors Retreat to:

  • identify areas of Board performance that are strongest and those that need improvement
  • identify priority areas for the Board to focus on in the next 1-2 years
  • allow different views to emerge, be expressed and considered

There are two parts: a self-assessment of individual members’ performance, and an assessment of the Board’s effectiveness and performance as a whole.

For each consideration, please rate your response on a scale from 1-5, with 1 as the lowest possible state, and 5 as the highest possible state. You may add comments after each section.
